What We Do

Our goal is to provide a safe, supportive place where you can rest, heal, and get the care you need to move forward.

We are a 24-hour care program.  We help you recover after leaving the hospital or a nursing facility.  We provide medical staff, case management, and support services.

How do I enroll?

How Long Can I Stay?

Your stay depends on your medical and support needs. Most stays are short-term. The longest stay allowed is 90 days.

How Much Does It Cost?

You will not have to pay out of pocket for your stay. Services are covered through your insurance provider.

Who We Welcome Into Our Program

  • Individuals leaving the hospital
  • Individuals leaving a skilled nursing facility
  • Individuals who need therapy, wound care, or IV antibiotics
  • Individuals who need ongoing medical support

 

 

Admission Guidelines

  • Must be homeless, at risk of homelessness, or have no safe place to recover
  • Must be medically stable
  • Must be able to take medication with staff support
  • Must be able to independently manage personal care and toileting needs
  • Must be alert, aware of their surroundings, and able to understand their current situation
  • No illegal drugs or violence allowed

Who May Not Be Eligible

  • Individuals with active contagious illnesses (active Tuberculosis, C-Diff, or MRSA of sputum)
  • Individuals who are medically or mentally unstable
  • Individuals actively detoxing until medically stable
  • Individuals participating in a duplicative state-funded program

  • Limited or short-term help with daily activities

 

Readmission

You may be able to stay in the program up to two times in one year. Approval for another stay depends on insurance and program review.  Medical clearance is required before readmission.
